Caroline Ambrose, from Westminster, survived the leap but died three years later from pneumonia and catastrophic brain injuries at the Royal Neurological Hospital in May last year.

Barnet Coroner’s Court heard at the time of the incident she was being treated at the Cygnet Hospital in Harrow for complex psychological problems.

Although she had self-admitted to the unit, her doctors considered her vulnerable and decided she should not leave the premises unless escorted by a member of staff.

But on May 2, 2011, a nurse August Mberi agreed to allow her to take a trip to the shops alone after she told him she needed ‘some fresh air’.

Mr Mberi told the inquest he believed she had been granted unescorted leave from the hospital the previous day and that she had coped well.

He added she had been calm and coherent when she asked him to leave the hospital.

Coroner Dr Andrew Walker said she should never have been allowed to leave the hospital alone and told the inquest: “This was a really serious failure to provide basic medical care to Ms Ambrose by allowing her to leave unescorted.

He added: “There seems to be no discretion allowed to Nurse Mberi, he had no discretion to do anything else other than what had been decided by doctors, regardless of her condition or the condition she appeared to be in.”

Following her accident, her brother, who won the 2007 series of reality TV show, ran the London Marathon to raise money for both the Royal Neurological Hospital and the Volunteer Service Organisation, for whom Ms Ambrose had worked in India.
